Wheeze and hypotension minutes after a meal

A 34-year-old develops diffuse urticaria, tongue swelling, wheeze and light-headedness 5 minutes after eating a dessert containing peanut. Blood pressure is 78/46 mm Hg and pulse 128/min. What is the best immediate treatment?

Which intervention should be given first?

Pharmacology in practice
GASTROENTEROLOGY

Omeprazole (oral)

Indication
Acid-related disorders, including reflux oesophagitis and peptic ulcer disease; H. pylori eradication only with combination therapy.
Mechanism
Inhibits gastric parietal-cell H+/K+-ATPase, suppressing the final step of acid secretion.
Interactions
Avoid with clopidogrel where possible: CYP2C19 inhibition reduces formation of its active metabolite; separating doses does not remove the interaction.
Important adverse effect
Common: headache, abdominal pain, diarrhoea, nausea or vomiting. Long-term use can cause hypomagnesaemia; reassess need and duration.
Dosing
Typical adult reflux oesophagitis regimen: 20 mg once daily for 4 weeks; indication, response and organ function determine regimen and duration.

Tozorakimab to Prevent COPD Exacerbations

Two replicate phase 3, placebo-controlled trials (OBERON and TITANIA) studied current or former smokers with COPD and a recent exacerbation despite stable inhaled maintenance therapy. Tozorakimab reduced annualized moderate or severe exacerbations in the overall populations: rate ratio 0.70 (95% CI 0.58–0.85) in OBERON and 0.71 (0.59–0.84) in TITANIA. Results were also significant among former smokers. The authors concluded that exacerbations were reduced; these trials establish an effect on exacerbation rate, not yet long-term mortality or universal suitability. Summary based on the accessible original abstract; supplementary methods were not reviewed.

Idiopathic PAH: confirm the haemodynamics before treatment

Diagnosis / Testing

Suspect pulmonary hypertension with otherwise unexplained exertional dyspnoea, syncope or right-heart findings. Echocardiography estimates probability; it does not establish PAH. Complete a structured work-up for lung and left-heart disease and associated conditions. Ventilation–perfusion scanning is recommended to exclude chronic thromboembolic PH; right-heart catheterization in an experienced PH centre confirms haemodynamics and classification.

Treatment / Care Decisions

The 2022 ESC/ERS definition of precapillary PH is mean pulmonary artery pressure >20 mmHg, pulmonary artery wedge pressure ≤15 mmHg and pulmonary vascular resistance >2 Wood units. Refer suspected PAH for expert assessment before PAH-specific therapy. In appropriately classified patients without cardiopulmonary comorbidities, initial endothelin-receptor antagonist plus PDE5-inhibitor therapy is recommended for low or intermediate risk; regimen and escalation are risk-based.

Risk Assessment / Prognosis / Epidemiology

Assess WHO functional class, exercise capacity, biomarkers, right-ventricular function and haemodynamics; right-ventricular failure drives clinical deterioration. Reassess after treatment initiation and during follow-up, ideally with a multiparameter risk tool. Pregnancy and planned procedures require specialist discussion because PAH carries substantial maternal and perioperative risk.

Pathophysiology / Basic Science

PAH is a precapillary pulmonary vascular disease with increased pulmonary vascular resistance. Endothelial dysfunction, vasoconstriction and vascular remodeling raise right-ventricular afterload; progressive right-ventricular dysfunction can lead to systemic congestion and low cardiac output. Treatment targets several pathways, including endothelin, nitric oxide–cGMP and prostacyclin.
